The CCG work in partnership with placing authorities to ensure, where possible, that objectives and outcomes for the children and young people we support are recognised before they come to stay with us.

 

Wherever possible, introductory visits prior to placement commencing will be arranged for the child/young person and relevant adults. In addition, a pre-placement meeting involving all relevant professionals and significant adults will be convened to establish that:

 

  • A transitional plan is place
  • The objectives & outcomes of the placement
  • Any timescales involved
  • All relevant paperwork is shared
  • All required risk assessments are in place
  • Information regarding health, education, safeguarding & developmental needs is shared
  • Existing external supports can continue

Emergency Placements

Planned admissions are preferrable but we recognise that this route is not always possible and that placements are often necessary at short notice. The CCG provide a 24/7 service and are open to emergency referrals if availability allows.

Out-of-hours emergency placement requests should only be made by telephone as emails are not continually monitored out-with office hours or weekends.

 

Following an emergency placement as with planned admissions, a post-admission multi-disciplinary initial review will be held within 72 hours to consider if the placement is appropriate and, if so, to establish long-term service requirements for the child or young person.
